• Beginning March of 2012, some of the following forms are available in several formats. While the forms are still available as a plain Microsoft Word or .pdf document, they are now also available as interactive Word and .pdf documents. It you have Adobe Acrobat Reader, you will only be able to fill-in the interactive .pdf forms on your computer to print, but will not be able to save them. If you use a Macintosh, you can open the interactive .pdf in Preview, which does allow you to fill-in the form, save and print. If you choose the interactive Word format you will need Microsoft Word on your computer to open the documens and to complete it on your computer. You always have the option of just printing the blank form and filling it in by hand.
