Membership Chairperson, Jean Jaeger is trying to get all of our membership papers and forms in order. To that end she has asked us all to fill out two forms, the Member Biographical Data Form (jokingly referred to as "write your own obituary") and the Member Profile Form (below the Biographical Data Form Information - scroll down).
The Biograhical Data Form is linked here to download in Word format. Here is what you need to do:
- Download the form
- Open it in Word and use "Save As" to save it with your name replacing the "LF-Member" portion of the file name (mine was "Jan Paxton Biographical Data for Chapter.doc") in some folder on your computer where you will be able to find it again later.
- Fill out the form in Word. The nasty little lines disappear as you type! Save it as you go so that you don't lose your hard work. Give all the details!
- Save the final version and remember where it is on your computer.
- Write a note to Jean Jaeger <jeanjaeger@optonline.net> and attach the final version of your file - your e-mail should have a little paperclip icon or a place that says "Attach File" that will allow you to browse your computer and find the file in that place you carefully saved the file.
- Send your note off to Jean once the file is attached. She should e-mail you back in a day or so to let you know that she got it. If she doesn't....something went wrong and I don't know what it is, but if you call me (201-851-1428) I'll try to help you sort it out. ~ Jan
Member Profile Form
The online form which follows replaces the paper version Jean gave out at the October meeting. Only those without computer access should use the paper form, as their data will need to be entered by someone later. Complete the form. When you are done, click the "submit" button. You should get a "Thank You" response. If you do not get a response, your form has not been entered. You have done something incorrectly. Try again or call me and I'll try to help youo figure out what went wrong and walk you through the correct steps. (201-851-1428) I'll try to help you sort it out. ~ Jan
