2011-2012 Recognition of Chapter Achievement form
The Chapter Achievement Form has been designed to recognize chapters for distinctive, excellent and superior achievement. Five (5) chapters who score at the superior level and earn the highest number of points will receive the Presidential Chapter Achievement Award. All chapters meeting the criteria for any of the achievement levels will be recognized at the 2012 Eta State Convention.
